NEW PATIENT INFORMATION

for choosing our office to care for your dental needs. We
are delighted to welcome you to our dental family and
appreciate the opportunity to provide you with quality
dentistry in a comfortable and relaxed setting. Our desire
is to enter into a partnership with you based on trust and
mutual responsibility. Please know that your dental needs
come first in our practice. We are committed to provide you
with quality, long-term dental care.
Our New Patient packet of forms is attached. To download
the New Patient packet,
click here. Please
print them out, read, and complete the forms. Bring the
completed forms with you to your appointment. Your
appointment time is reserved especially for you and we
request that you arrive on time. For your first
appointment, however, we request that you arrive 15 minutes
early to allow us to enter your information into our system
before you are seen.
Unless prior arrangements are made, payment is due at time

Financial
Arrangements
We are happy
to submit insurance claims for you electronically and to
have the check come to you within 4 weeks. We will
therefore expect that all treatment will be paid for at the
time services are rendered unless you have established
credit with our office.
We accept
Visa, Mastercard, American Express and
Discover Cards.
As an
alternative, you can authorize the check to come to us,
however, you must have established credit with our office.
To do so, we will ask that you assign us to collect the
unpaid insurance balance from your credit card.
